Premium Suite (Suite) consists of a combination of any Performance or Signature level checking account (Checking) and a Performance Savings account (Savings) or a Titanium Money Market Savings account (TMM) opened on the same day. Promotional offer consists of your choice of: an Amazon KindleTM with an eBook (Reward) or an account credit and a book (Credit). Must be selected at Suite opening and is available to new consumer customers only. Offer valid for Suites opened 1/1/11 – 3/31/11. Opening deposits may not be transferred from an existing MidFirst account. Limit one (1) promotional offer per household. All accounts subject to approval. Must be 18 or older. Offer subject to change without notice and is not valid with any other offers. Minimum opening balance required to qualify for Reward or Credit on Suite: Checking-$100; Savings-$500; TMM-$5,000. Annual Percentage Yields (APY) vary by account type and minimum deposit amount. For example: A Performance Plus checking account with a $100 opening deposit has an APY of 0.05%; APY is accurate as of 12/10/10 and is subject to change without notice. Fees may apply and reduce earnings. Requirements to earn the Reward or Credit must be completed on the Checking within 75 days of Suite opening and are as follows: 1. Setup $250 minimum monthly recurring direct deposit; 2. Complete twenty-five (25) Visa® Check Card signature-based transactions; 3. Elect eStatements and complete three (3) bill payments to a third party vendor through our Online Banking. Customer must meet all requirements above in order to receive the Reward. If Credit is chosen each activity will be awarded individually; $50 for opening the Suite and $25 for each of the three requirements (numbered 1-3) met. Customer will receive Credit(s) or Reward within 90 days of Suite opening. Customers will receive a 1099-INT for the value of the Credit or Reward (up to $189) received. Suite must be open and active at the time of Credit or Reward. Accounts closed within ninety (90) days of the Suite opening are not eligible for the offer and may be subject to charges equal to the value of the offer and early account closure fee. A monthly fee of $2 will be applied to Free Checking Plus accounts enrolled in iManage Personal Banking® that choose to receive paper statements.
